Savannah has developed unique programs under its ten-year plan: the Homeless Continuum of Care and the Behavioral Health Continuum of Care are now linked and the police department created a Crisis Intervention Team model. The CIT establishes front door treatment and improves the delivery and accessibility of services, such as access to employment and benefits. To decrease chronic homelessness, case management, outreach and permanent supportive housing are required. To ensure success, a data collection system will measure the plan's results.
