The Leadership Council to End Homelessness on Cape Cod and the Islands published “A Ten-Year Plan to End Homelessness on Cape Cod and The Islands” in February 2005. In order to actualize the vision to end homelessness within ten years, a combination of action steps will be needed such as strengthening existing services, including prevention; developing new programs and services where needed; and developing more housing, namely Housing First Models. The elements of the plan include prevention, discharge planning, coordination of existing services, and housing development.
