This survey of consumers (people experiencing homelessness or that formerly experienced homelessness) can be used as an evaluation tool to determine if coordinated assessment is creating a shorter path for consumers between homelessness and a return to permanent housing.

Communities can use this tool as a quick way to assess how well their coordinated assessment system is functioning. The tool has two parts: one part should be completed before a coordinated assessment process has been implemented, and one part should be completed six months to a year after implementation.
